One-Time Enrollment Fee (based on company size)

Family add-on enrollment fee: $20 per person (can be employer-paid or employee-paid).
Other Important Details

12-month agreement, auto-renewed annually

Employees can be added or removed anytime

Billing occurs monthly, based on employee count on the 26th of the prior month

Choose billing on the 1st or 15th

Plans starting mid-month are prorated
