? Four Telehealth Urgent Care Visits Per Year

No extra cost.
Additional visits are just $35, that is less than the cost of gas and frustration from sitting in an urgent care waiting room.

• Household/family (up to four members, ages 6+): +$175/month

• Additional family members: +$25 each

Please note: pediatric ongoing care, scheduled immunizations, and OB/GYN services are not included.

Contract Terms & Billing

We keep it simple, transparent, and flexible.

One-Time Enrollment Fee (based on company size)

• 1–25 employees: $250

• 26–50 employees: $400

• 51–75 employees: $550

• 76–99 employees: $750

• 100+: $1,000

Family add-on enrollment fee: $20 per person (can be employer-paid or employee-paid).

Other Important Details

• 12-month agreement, auto-renewed annually

• Employees can be added or removed anytime

• Billing occurs monthly, based on employee count on the 26th of the prior month

• Choose billing on the 1st or 15th

• Plans starting mid-month are prorated
